On behalf of the Student Success Center, we hope this page is helpful for our advisors. It includes forms and procedures for advising that we hope are beneficial as you work with your students.

Forms

• Change of Major Form — Online form to completed for official changing/adding a major and adding/dropping minor(s). Student must provide information on their current Academic Advisor and (if appropriate) Coach.
• Advising Agreement Form — Form to complete for approval by student’s academic advisor.
• Liberal Arts Core Requirements — Source to compare student transcripts for assistance with selecting upcoming semester courses.
• Academic Recovery Plan — Official form for completion by Primary Academic Advisor with advisee in designated academic standing.

• Advising Manual — Comprehensive reference guide to assist Advisors when advising students
• Student Success Quick Links — Quick access to all on-campus and online resource links
• Add/Drop Class — Online form to complete for schedule changes during semester add/drop period
• Academic Calendar — 4-year outlay of important academic dates (Add/Drop, Commencement, etc.)
• Current Catalog — Full description of academic program requirements and courses
• Campus Resources — Contact information and departments for academic support, financial & degree inquiries, IT assistance, Wellness and Student Services
• MyCN — Direct link to search and register for classes (requires C-N login using single sign-on)
• ACE — Early Alert Referral System to address high-risk behaviors in undergraduate students
