Recreational Services & Intramurals Survey

Thank you for your participation in a recent Recreational Services or Intramural event.  Please take a moment to fill out the following survey so that we may better evaluate our strengths and weaknesses. 

Submissions will be entered into a drawing for one of five $10 gift cards to the Creek Cafe or one $50 gift card for Walmart!  If you would like to be entered into the drawing, please be sure to include your name and email address in the optional fields at the top of the form.  Gift cards will be awarded at the end of the semester and winners will be notified by email.
